Inflate your long, thin balloon to full size, then release some air. The idea is to loosen the tension of the rubber so that it can be pinched and twisted in the process of forming your kitty ears.
Create a bubble that is 1 to 1 1/2 inches in size. Hold a long, thin balloon in both hands. Place your hands about 2 inches apart. Twist the balloon so that it forms two inflated sections, linked by a pinched section of rubber.
Change the position of your hands so that one hand is holding the bubble you just created, and the other is on the remaining length of the long, thin balloon. Twist the balloon again so that the bubble is completely separated from the remaining length of balloon by two pinched or twisted sections of rubber.
Grab the two pinched sections surrounding your bubble and twist them together. This can be accomplished by holding the two remaining legs of your long, thin balloon, and turning the bubble around atop them. Once twisted, the small bubble will take on a curved, peaked, ear-like appearance.
Perform the same process again with the same balloon. With a little practice, you will be able to properly space two kitty ears.
