Enlist the assistance of the parents. They will or should know what the child will respond to the quickest. Ask them to stand directly behind you while they are trying to get the child to smile. This way, when the child does smile, she will be looking toward the camera.
Attach a remote release to your camera and turn the drive to continuous. This will allow you to stand out from behind the camera. Make eye contact with the child and try to get him to smile by using sounds or facial expressions. When the child smiles, press and hold the shutter release to take a few images right away.
Use keys or other shiny objects that make sounds. Jingle the keys or other items until the child responds. Press and hold the shutter release to take a number of pictures in succession, which should yield a few good pictures to choose from.
