Add 1 inch to the height and width you want the pouch to be.
Cut four rectangles of fabric with these dimensions.
Pin two of the rectangles together with the right sides touching and all edges even. Pin the other two rectangles together in the same way.
Sew around the edges of the rectangles with a 1/2-inch seam allowance, using a straight stitch. Make square corners by stopping at the corner with the needle down in the fabric, lifting the pressure foot and turning the fabric 90-degrees before lowering the foot and continuing the seam. Leave a 3-inch opening for turning.
Add 2 inches to the height dimension and cut two rectangles of fabric with that dimension and the same width as the other rectangles. Sew these two rectangles together in the same way.
Clip the corners of the seam allowances at a 45-degree angle. Turn the rectangles right side out and press them.
Stack the rectangles with the large one on the bottom. Stack the other two on top with the bottom and side edges even. The extra fabric at the top will be the flap on the pouch. Pin the stack of fabric together.
Stitch the stack of fabric together 1/4-inch from the edge. Stitch around the flap at the top also. This will top-stitch the flap so that it matches the rest of the pouch.
