Label the spreadsheet. On the column for titles, assign the question number. For the top row, indicate the survey number.
Write down the number in the spreadsheet that corresponds to the response made on the survey. Write down one response number per row.
Add the responses up for each survey response. For example, if one respondent circled the numbers 1, 4, 3, 5, 5 in their survey, you would add them all up. Note that some questions may be counted as reverse points, meaning that if they were marked as one, it should be the opposite, five. Likewise, if it were 2, then 4. Your survey leader should inform you which questions you should reverse.
Place the total sum of the responses in the row below the responses for that survey.
