Unlocking the Level
To unlock the secret level, you must beat the game on any difficulty. Once you do, there are two ways to access the stage. The first is to simply wait out the ending credits until the end, when the stage will automatically start. The second, if the game has already been completed and the credits were skipped, is to use the Mission Select option at the main menu, and select the newly added mission "Epilogue."
Mission Overview
In "Epilogue," or "Mile High Club," as it is also called, your mission is to rescue a VIP from terrorists aboard a passenger jetliner in flight and extract him to safety. You and your squad start at the rear of the craft's lower level and must fight your way through infinitely re-spawning enemies all the way to the conference room at the front of the plane. While this is, aside from the training mission, the smallest and simplest stage in the game, it is made difficult by the time limit: You have exactly three minutes to reach the VIP and free him; if you take too long to move forward, enemies will respawn.
Reaching the Hostage
Throughout the mission, try using either a P90 or the silenced MP5, and keep all weapons fully loaded to save time. After you take down a terrorist exiting the toilet, take the right-hand path up the aisle, as it will provide you with more cover as you advance to the front of the plane. After clearing the lounge before you, check your weapons, then proceed onwards. Throw a flashbang into the next lounge. When it detonates, rush in and clear the room, quickly making your way to the stairs. At this point, the cabin will depressurize and the plane will roll slightly, tilting your point of view. Repeat the process of flashbanging and clearing the third and fourth lounges. When you see a pair of double doors, run toward them before the time runs out.
Extracting the Hostage
They will open to reveal one last terrorist holding the VIP as a human shield. You will have five seconds to either carefully shoot the terrorist in the head or to hit his leg followed by a shot to the head before he can execute his hostage. Quickly enter the room. You will find that the terrorists have armed a bomb aboard the plane, and a 30-second countdown to its detonation will start. Assist your teammate in breaching the hatch, then leap out of it before the timer reaches zero to complete the mission. If you successfully do so under the hardest or "Veteran" difficulty level, you will be awarded the "Mile High Club" achievement.
