Remove the ink tube from the ballpoint pen. Most pens can be dissembled easily, and you only need the central ink reservoir that contains the tube and attached ballpoint tip. The rest of the pen can be thrown away.
Remove the ink from the reservoir. Preferably, you want to use a pen that has depleted all its ink, but removing excess ink from a ballpoint reservoir is not difficult. You can easily pull the ballpoint off the end, and slid a thin toothpick through the tube. This will dislodge the barriers holding the ink in place. Wash the ink out and leave the tube to dry for awhile.
Reattach the ballpoint tip onto the tube. Most ballpoint tips can be pushed back in place simply by sliding the metal back into the tube. If the tip won't stay in-place, just use a dab of glue.
Cut the tube to an appropriate stylus size. To measure the tube, slid it into your DSi's stylus slot and cut the excess plastic at the point where the tube protrudes from the DSi. This will shape the tube to fit your DSi.
Wrap scotch tape around the blunt end of the tube. On the end opposite the ballpoint tip, wrap a lengthy strip of scotch tape around the tube. This will pad the tube, allowing better grip and usage of the makeshift stylus. Additionally, the padded scotch tape will make it easier to pull the stylus out of the DSi's stylus slot.
