Disc Troubleshooting
Flip the disc over and examine the quality of the burn. There should be a dark, purple ring on the bottom that nearly covers the surface. Make another copy if you see rings or dots on the surface, as if data is missing in certain areas.
Set the write speed to "4x," or "3x" if you have this option. The Wii reads games at 3x, plus the slower speeds produce copies with fewer errors.
Clean the disc thoroughly with rubbing alcohol and a lint-free cloth. Let it air dry before placing in the console.
Download a different version from the Internet or make a new backup on your computer. The version you have may be corrupt.
Use a Wii that has a mod chip, or that is software modded or the game will not play.
Burn the ISO using high-quality media, like Taiyo Yuden or Verbatim.
USB Troubleshooting
Check your USB device to see if it is hot. Overheating from long-term usage can cause the game to crash.
Unplug your LAN adapter to see if it is interfering with the USB loader.
Install a different loader through the SD card and Homebrew if you have a soft modded Wii. Some loaders work better with specific games.
Insert a disc in the Wii before launching the game. It is possible the game requires a disc, even when booting from a drive.
Check the compatibility table at gbatemp.net to see if there are any reported issues with your version. The PAL version, for example, has issues playing sound on firmware 3.2E.
Change the video mode option in USB Loader to match the region of your Wii. For example, set it to "Force NTSC" if you live in the U.S., Canada or Japan.
