Start a new game on "The Boss Extreme" difficulty. Lower difficulties will not award the Big Boss emblem even if you meet all the requirements. For an easier time, begin this game with a good stock of Drebin Points and weapons built up from previous games.
Activate no alert phases the entire game. If you get an alert at any time, you'll have to reload from an earlier save. Take your time to avoid enemy detection.
Kill no human enemies throughout the game. You'll have to stick exclusively to non-lethal methods the entire time -- Close-Quarters Combat and tranquilizing guns. This includes bosses as well, except for Vamp and the first form of each Beauty. You can destroy tanks and other non-human enemies without missing this goal.
Use zero continues during your game. The game has no limit on the number of times you can save, so save often and reload a save if you lose instead of continuing.
Eat no healing items at any point. This goal, though tricky, doesn't make a huge difference given that you have to go unnoticed by enemies throughout the game anyway. If you do take some damage, it will heal slowly on its own while the game is off. Accelerate this process by setting your PS3's clock ahead a few hours.
Avoid using the stealth camo and bandana special items earned from previous games. Equipping either of these will prevent you from earning the Big Boss emblem.
Finish the game in less than five hours. Combined with staying hidden, this presents a major challenge. Nearly all game time counts toward this limit, including cutscenes and codec calls, so skip them all. You can't skip the final 16 minutes of cutscenes in the game, so budget your time accordingly.
