Locate the five screws on the rear of the PlayStation controller and use your Phillips screwdriver to remove them.
Lift the back casing off of the controller. You will now see the battery pack resting on top of the green controller circuit board. Lift up the battery and set it to the side of the board.
Locate the large black square chip in the center of the circuit board. This is where you will attach the mod chip.
Peel the protective cover off of the underside of the chip to expose the adhesive.
Place the mod chip over the chip on the circuit board. The mod chip will stick to the circuit board chip.
Place the battery over the circuit board and replace the back cover.
Replace the screws, using your Phillips screwdriver.
