Instructions
Open your web browser and navigate to the official Sony PlayStation Support website (see References). Click "Service." You will be taken to another screen. Click "Service" again.
Select the system you want serviced. Your choice will be either the older or newer PS3 (each represented with its own box) as it is the only console compatible with the Sixaxis controller. Select your country, and you will be taken to another screen.
Use the drop-down box to select your model number. Type your serial number in the box below. Both are located on the back of your console if you have trouble finding it. Choose "Yes" or "No" based on whether you have a proof of purchase. Select "Continue."
Type in a description of your Sixaxis controller problem in the box provided. Type in the numeric code at the bottom of the screen and select "Enter." You will be taken to another screen where you will be quoted a price depending on whether your system is out of warranty. This will vary based on the extent of the repairs.
Enter your personal information, credit card number and shipping details. The company will mail you a box to send in the controller. The wait time varies based on how many requests Sony has.
Mail the controller back to Sony. Once the repairs are complete, your credit card will be charged and the controller will be mailed back to you.
