Remove the controller's batteries and install fresh ones. Many controller faults are due to low-battery issues, which can cause sporadic functionality.
Reset the Xbox 360 with the "Halo 3" disc in the tray. Wait for "Halo 3" to reboot and reconnect your controller by pressing its "Xbox Guide" button (the green-on-silver "X" in the center of the controller).
Make sure that any battery packs, such as the Xbox 360 Rechargable Battery Pack, or the Xbox 360 Play &Charge Kit--are connected firmly and charged fully. Check the charge of the pack by holding the Xbox Guide button and watching for the charge indicator in the upper-left area of the Guide.
Make sure that your Xbox 360 is operating in a cool, dry area--free from dust or direct sunlight. The vents on the back of the console must be able to operate freely. If necessary, move the console to a more optimal location to reduce operational errors.
Check that your "Halo 3" game disc is clean and free from marks. Clean the disc with a clean, lint-free cloth that is lightly sprayed with glass cleaner. Clean in straight lines from the center to outer edge of the disc, never in circular motions.
Try a second controller on "Halo 3." If another controller works but the first does not (even after performing all the above steps), your controller may be faulty. Contact Microsoft to arrange a repair or replacement.
