Don't read the Xbox Code of Conduct. There's nothing important in there anyway. The code prohibits activities such as giving out personal information, posting lewd material, and impersonating Microsoft personnel.
Be rude to others online. Curse, flame, argue. All of these are acceptable. During online games, using the microphone to cast insults or using a camera to expose yourself are good ways to do this.
Ask people from in the game to meet out outside of the game. Choosing to meet people out in real life can be extremely unsafe. Moderators who see players trying to do so will certainly ban you.
Cheat. There's probably no one paying attention anyway.
Use a modified Xbox. Even if you know you're playing with modders, you can get banned too, so look for them on servers. Anyone who benefits from modders or cheaters is subject to banning themselves.
Spamming is another great way to get banned. Friend as many people as you can, then send them messages and game invites constantly. Be as annoying as you can and you'll be sure to be reported.
